Below are a case opening booklet and medical release form, developed by the Youth Advocacy Project, as well as two forms that help lawyers, parents, and clients understand the Juvenile Court and Probation processes.

JDN's Case Opening Booklet (updated in September 2007) is a form that helps lawyers obtain necessary information about their clients that will help organize and familiarize lawyers with their client's individual needs.

The Release Form is a medical disclosure form that authorizes lawyers to legally gain protected health information about their clients.

"Now That You're In Juvenile Court..." explains how the court process work for juveniles. This sheet includes the roles of the prosecutor, probation officer, lawyer and judge as well as the process of arraignment.

"Now That You're On Probation..." (in .pdf format) explains the responsiblities of the probationer and includes tips for staying out of DYS after the term of probation has been completed. Please feel free to download these as well.
